Piperacillin/tazobactam resistance in a clinical isolate of Escherichia colidue to IS26-mediated amplification of blaTEM-1B

We identified a clinical isolate of Escherichia coli displaying an unusual, emerging phenotype; piperacillin/tazobactam (TZP)-resistant, 3rd generation cephalosporin-susceptible. Prior to treatment with TZP, TZP-susceptible E. was isolated from the same patient. Hyperproduction class A β-lactamase has previously been linked this phenotype, but mechanism hyperproduction in isolates lacking promoter region mutations is not well understood. Clonality two initially assessed RFLP, and activity determined using nitrocefin assay. Both were sequenced on Illumina Oxford Nanopore Technology platforms fitness competitively. plasmid construct containing insertion sequence IS26was used capture translocatable unit (TU) vitro. The confirmed be clonal, TZP-resistant hyperproducing blaTEM-1. However, no isolate. Hybrid assembly revealed that ~11kb segment DNA excised IS26 flanked pseudo-compound transposon isolate, forming circular TU Multiple re-insertion events TU, mediated by IS26, led tandem repeats within chromosome, increasing copy number Excision via TU. Amplification incurred significant change different environmental conditions. This study improves understanding TZP-resistant, cephalosporin-susceptible phenotype antimicrobial resistance prediction genotypic data.
